Pet Drop-Off Consent Form

While every cat is given the individual attention they require during their procedure, please keep in mind the reduced cost is due to the high volume of this clinic. If your cat has special needs, is stressed easily, is immunocompromised, etc, this may not be the best option for them. Cats will be exposed to many animals and other stressors during the trip and while in the clinic receiving care. Cats of Chase is happy to facilitate low-cost spay/neuters, vaccinations, and other services, but owners must accept responsibility for the mild risks that exist.

Each cat must arrive in their own carrier (one cat per carrier). All TNRs must be in a live trap.

Thank you for dropping off your pet with us today! The following information will be used to help our veterinarians better care for your pet.
Reason for Visit (Select all that apply)

You will be contacted prior to pick-up, which usually occurs between 3 and 5pm. Cats cannot remain overnight unless specific arrangements have been made ahead of time.

I authorize anesthesia/sedation for my pet. I understand some risks always exist with anesthesia and/or sedation. As the owner, or agent of the owner, of the animal above, I hereby give Magnolia Spay Neuter Clinic consent to perform surgery and/or treatments for my pet. I understand that during the performance of this procedure, unforeseen conditions may be revealed that may necessitate an extension or variance in the planned procedure. I expect Magnolia Spay Neuter Clinic to use reasonable care and judgment in performing the procedure(s). While I expect all procedures to be done with the best of the abilities of the professional staff, I realize that no guarantee or warranty can ethically or professionally be made regarding the results or cure. I understand that I assume financial responsibility for all services. Payment is due in full at the time services are rendered.
